INSTRUCTIONS

Please complete the attached forms before the first day of school to ensure your student's continuous enrollment at FCA Frisco for the 2020-2021 school year.

PDF FORMS:

On-Campus families please print, sign and hand in the following attached forms when you pick up your car tag:

• Socioeconomic Form

• 2020-2021 Family Survey Form

Virtual families please print, sign and hand in the following attached forms at your assigned resources/textbooks pick-up day and time:

• Socioeconomic Form

• 2020-2021 Family Survey Form

Virtual families who requested Chromebooks/Hot Spots have to complete the Student Chromebook Checkout Agreement Form and hand it in at your assigned resources/textbooks pick-up date and time.

GOOGLE FORMS:

All families - Google forms need to be completed and submitted before the first day of school.

If your child has prescribed medicine they take, please complete the attached Medical Administration Permission Form and drop it off at the office in Building A before the first day of school.

In an effort to reduce the risk of Covid-19 exposure, ALL VISITORS must complete the attached Symptoms Screening Form.

The Voluntary Student Insurance form is attached for your information.
